- Statement under penalty of perjury: Confirm that the information in your notice is accurate and that you are the copyright owner or authorized to act on the copyright owner’s behalf.
- Signature: Include a physical or electronic signature.

Counter-Notification
If you believe that your content was mistakenly removed or disabled due to a misidentification or error, you may submit a counter-notification. To do so, please provide the following:
- Identification of the material that has been removed or to which access has been disabled.
- Your name, address, telephone number, and email address.
- A statement that you consent to the jurisdiction of the federal district court for the judicial district in which your address is located, or if your address is outside of the United States, the jurisdiction of the federal district court for any judicial district in which the service provider may be found.
- A statement that you will accept service of process from the party that filed the notification or an agent of such party.
- Your physical or electronic signature.
